The LASIK network gives members excellent access to approximately 600+ provider locations nationwide. Members and their eligible dependents are entitled to significant and exclusive savings. In addition, providers must meet our stringent credentialing standards to serve members.

Participating Lasik Centers Look up
Step 1:
Locate a participating provider by visiting: http://www.careingtonlasik.com/#/find
Step 2:
When scheduling an appointment, please inform the participating provider’s office that you are a member of National Lasik Network.
Step 3:
Please present your Simple Medical Benefits ID card upon arrival to receive savings.
Step 4:
The provider will collect payment at the time of service. You are responsible for paying the total bill, less the applicable savings, when service is provided.
